If you want to enroll an unregistered domestic partner in your health plans, you must complete the Domestic Partner Enrollment Affidavit.

An employee benefits representative will open a life event and send you an email with instructions to make your elections. receipt of your paperwork will start the life event process, but you must then log in to actively make your new elections after receiving an email notification of employee benefits.
